General practice at the heart of change

In Manchester, ten local care organisations will form a major part of the fabric of the £6bn devolution programme. General practice is driving the development of these organisations. Elsewhere in the country, PCC is helping an emerging multi-specialty community provider (MCP) to sort out pay for GPs who sign up. Every item in the current issue of Commissioning Excellence focuses on the role of general practice in the changing healthcare landscape – not discussing the theory but real developments that are happening now. A group of practices in Larwood, Nottingham, has already reduced unscheduled admissions from local care homes by 67%, part of a £290,000 annual saving in urgent care costs. This issue also reviews PCC’s recent Contracting for new care models event (it runs again in Warrington this week), which gets under the covers of ACO contracts. Commissioning Excellence looks too at training initiatives to help practices better manage workload while improving the service for patients.

Paying for IAPT case studies

NHS England has published case studies based on the experience of five organisations (three CCGs and two providers) on developing and implementing a new payment approach for improving access to psychological therapies services, which link a component of payment to quality and outcome-measures. The case studies may help commissioners develop their own outcomes-based payment approach. As local pricing Rule 8 of the 17/19 National Tariff requires organisations to adopt an outcome-based payment approach by April 2018, these case studies could be of particular value at this time.

Advance care planning for people with dementia

NHS England’s dementia team has just published a guide to advance care planning for people with dementia in all care settings. The dementia challenge 2020 maintains that “by 2020 we would wish to see… All people with a diagnosis of dementia being given the opportunity for advance care planning”. NHS England developed the guide with input from dementia experts, carers, health and social care professionals.

Developing new models of care in the PACS vanguards: a new national approach to large-scale change?

A report was commissioned by NHS England as part of a package of support provided to primary and acute care system (PACS) vanguard sites by the King’s Fund. The PACS model is an attempt to bring about closer working between GPs, hospitals, community health professionals, social care and others.

As the national new care models programme comes to a close, those who have led the development of the PACS model were invited to reflect on the process of being part of the programme, and of trying to bring about complex change in local systems. This report is not an evaluation of the PACS model or of the new care models programme, but it offers a unique set of first-hand perspectives into the experience of those leading a major programme at the national level and those living it at the local level.
